Important Instructions
Click on the Candidate Questionnaire link at the bottom
of this page and another page will appear. Click on that
link to see the questionnaire page.
At this time, the Candidate Questionnaire cannot
be submitted by email. One can fill it in, print, and
mail to:

or fill it in, print, scan, and attach to an email:
icdp@sbcglobal.net
We are trying to figure out what is causing the problem.
If anyone can help let us know please.
Also, the questionnaire is in .pdf format. Those who have Adobe
Reader installed on their computer will be able to access the
questionnaire. If you do not have Adobe Reader you can use the
following link to install it on your computer.
